Senior Manager, Transition of Care

Description UPMC is currently hiring a Senior Manager, Transition of Care! This is a remote position with potential in-office requirements as needed at 300 Northpointe Circle. This is a Monday-Friday Daylight position. Purpose:
This position will have oversight of the home health Transitions of Care and Advanced Illness Care programs. This position will interact with primary care providers and other health professionals in providing holistic care for seriously ill patients. Follows established advanced practice nursing standards, procedures and practice guidelines related to the care of the seriously ill patient in an acute care setting. Focus of care directed toward the physical, psycho-social, spiritual, existential, and bereavement management of the patient and family. Responsible to provide operational management for the Palliative Care Program. Assumes a leadership role in the development and ongoing improvement of the Palliative Care Program and customer relations, particularly with the medical staff, resident staff and hospital departments. Establishes goals and objectives which support and advance the Palliative Care Program and ensures appropriate actions for goal achievement. Develops and implements organizational strategies to integrate relevant philosophy and mission and values of the organization into the work environment.
Responsibilities:
Ensure development of and delivery of appropriate educational initiatives within the hospital and other settings. Maintain an evidence based approach to continuous program improvement. Responsible for integration of the program/service into the primary functions of the organization as well as coordination and integration of associated interdepartmental and intradepartmental services. Identifies, develops and participates in process improvement opportunities within the home health agency that will enhance the quality of the services we provide. Focuses on customer service and continually strives to perform the duties of their job in a manner that will result in optimal patient satisfaction. Discuss legal and existential issues including living will, power of attorney, code status, surrogacy, financial issues and end of life issues. Leadership in developing and implementing appropriate measures of patient care quality and satisfaction, such as pain and symptom management tracking tools. Meet with patients and families as necessary to facilitate discussion of patient and family goals. Oversees recruitment, employment and supervision of professional personnel. Function as a resource to staff for clinical and other palliative care and end of life issues that arise. Proven ability to facilitate change. Attend compliance training and adhere to the organization standards of conduct, policies and procedures. Assists with data collection, analysis, and quality assurance as requested.Responsible for initiating the plan of care for patients, provision of clinical oversight of care delivery and provision of expert direct care as needed in order to assure quality services. Provides education on palliative care including in-services for nursing staff and all health care professionals; conducting seminars for ancillary services to promote palliative care within the community. Completes performance appraisals. Maintains contact with Human Resources, payroll, Work Partners to facilitate personnel issues per agency policy and the law. Maintain quality control programs and continually assess and improve the quality of care and services rendered. Provide oversight and clinical supervision of the work of palliative care services, including team building, mentoring, role modeling and skill development. Coordinate and provide leadership to the interdisciplinary patient care team in active cases and new referrals. Facilitates program growth and development within the Transition of Care and Advanced Illness Care program specialty areas which enhance quality improved outcomes, emergency room visits, and greater patient satisfaction. Develop, review, and coordinate clinical processes that support appropriate referral and high quality, consistent provision of care. Coordinates and facilitates transition of patient care to hospice organizations where appropriate. Responsible for working closely with physicians and staff to ensure that palliative care services are well integrated with existing capabilities and educational and quality initiatives. Serves as a member on agency committeesPerforms duties and job responsibilities in a fashion which coincides with the service management philosophy of the organization towards patients, visitors, staff, peers, physicians and other departments within the organization. Monitors adherence to the budget and ensures adequate staffing and productivity requirements. Participate in self-development and continuing education programs on an ongoing basis. Participate in and represents the organization in community services to advance palliative care and end of life issues. Provides overall direction and co-ordination of the organization's Transitions of Care and Advanced Illness Care programs. Responsible for the development and implementation of policies, procedures and program documents that guide and support the provision of Palliative Care Services including pain and symptom management. On occasion will assist with discharge planning among health care professionals, medical services, social workers, case manager, the primary medical services consultants, and other health professionals and agencies involved with the care of the patient and family, home hospice and palliative home care. Qualifications Baccalaureate Degree required Masters Degree in Nursing preferred Hospice and Palliative Care Certification preferred Five (5) years nursing experience required Clinical nursing in oncology, hospice, geriatrics preferred Three (3) years management experience required Licensure, Certifications, and Clearances:
